and Cancellation Policy
Our goal is to provide quality medical care
in a timely manner. In order to do so we have had to implement
an appointment/cancellation policy. The policy enables us
to better utilize available appointments for our patients
in need of medical care.

Cancellation of an Appointment
In order to be respectful of the medical
needs of our community please be courteous and call our office
promptly if you are unable to attend an appointment. This
time will be reallocated to someone who is in urgent need
of treatment. This is how we can best serve the needs of our
patients.
If it is necessary to cancel your scheduled
appointment we require that you call by 10 a.m. one (1) working
day in advance. Appointments are in high demand, and your
early cancellation will give another person the possibility
to have access to timely medical care.
How to Cancel Your Appointment

. If you do not reach the receptionist you may leave a detailed
message on the voice mail. You may not cancel via email.
Late Cancellations
Late cancellations will be considered as a “no show”,
unless the appointment was made within less than 24 hours.
No Show Policy
A “no show” is someone who misses an appointment
without canceling it by 10 a.m. one (1) working day in advance.
No-shows inconvenience those individuals who need access to
medical care in a timely manner.
A failure to present at the time of a scheduled
appointment will be recorded in the patients’ chart
as a “no show” and a fee of $30.00 will be billed
to the patient.
Walk-In Clinics
We realize that illnesses and accidents
often occur unexpectedly. In an effort to provide the convenience
of timely medical care we offer walk-in clinics. Walk-in clinics
are for urgent and unpredictable medical care. You may experience
some delay as we try to accommodate you to our schedule.
Unless you have a booked appointment, all
health care is provided on a first come, first served basis.
All emergencies will be given priority.
New Patients
As a new patient, we will ask that you fill
out a brief questionnaire for our medical records. Your answers
will enable us to give you a complete examination.
Your Examination
Your examination takes place in a modern
fully equipped medical setting. A highly trained medical staff
member who will make you comfortable will escort you. They
will conduct a brief interview about your past health and
discuss with you your current discomfort. Dr. Yasmeen Jalal
will review the information obtained by our medical staff,
examine you, and after evaluation of the findings, he will
then explain to you the best procedure to follow to minimize
your discomfort.
About Children
Children are excellent patients if parents
help in preparing them for an office visit. The visit should
be treated as a part of the routine in his or her young life.
Every effort is made to avoid pain. Do not bring other children
to the examination if at all possible. A parent of guardian
must accompany children under the age of 18.
Calling Our Office
The receptionist or nurse will receive calls.
They have been trained to answer most questions. If the doctor's
opinion is required, you will be advised to expect a return
call. Please remember that many diseases have similar symptoms;
for this reason, most requests will require an office visit.
Medication cannot be prescribed for a medical problem that
has not been recently evaluated. For refills of medication,
please have your pharmacy call the office only during regular
office hours. Please always keep your pharmacy phone number
handy.
